South Dakota's nine federally recognized tribal nations include populations with high rates of informal caregiving for elderly tribal members and family members with chronic conditions. Tribal health organizations with caregiver support programs or existing elder care infrastructure are eligible applicants. The initiative targets long-term care and home health settings where tribal programs often operate. Community Health Representatives (CHRs) in tribal programs frequently serve caregiving support functions.
Application Guide
26RFP-26-09RHT-004 funds caregiver support programming under Initiative 8 (Strengthening Chronic Disease Management). The program provides peer support groups for caregivers of patients with chronic conditions, coordinated resources connecting caregivers to respite, financial, and community supports, and expanded provider training to help healthcare workers support the caregivers of their patients.
Rural caregivers in South Dakota — many caring for elderly parents, spouses with chronic conditions, or family members with disabilities — often lack access to the peer support and resource networks available to urban caregivers.
